My completed form is now entirely blank! Even the questions do not appear

I created a form "Personal Training Consultation Questionnaire" and then I decided to edit the form to include conditionals but now the entire form is blank (questions do not appear).

I went into the form history and discovered 74 updates were made where fields were removed (none of which I did) so I just tried reverting back to an earlier version before those changes and the earlier version is also blank.
