I've cloned your form for testing purposes. The widget seems to work fine, can you re-add the widget again and add the API key.
The problem might be there's a change in your API key or an update was made, please do re-add the widget and add again the API key. You can also find your API key in your Account settings, from there you can copy and paste the code to the widget.
Also, please do clear your form's cache. I hope this helps. Let us know if you need more assistance on this.
