Other PDF file showing on Smart PDF

Hi. I am trialing your program and created a form. All looks okay on my form, but it is somehow connected to some other form that is not applicable. When I complete my form (testing), it shows me a preview of this other form and then that is what shows up in my inbox. See image. My form is on the left, but this weird form is on the right.

Pls tell me how to get rid of this weird chiropractic form so I can use my own

It seems your form was set to Private because I am getting the message to get permission from you:

Kindly change the form to Public, here is your guide on How To Change the Access Settings of a Form.

I changed it to public. Can you now see the Body in Balance Chiropractic form? How do I get rid of that? When someone fills this form out, that shows up in the PDF preview window and it what I am receiving when I download it. I don't understand why this is connected to my form?
