I logged into today and my form is missing

    I logged into today and my form is missing.

    It is no longer listed underneath "myforms"

    Can you please fix asap?

    Hi,

    Upon checking your account, under the username cpmcInformatics, I see that there are 3 forms there. 

    This one though http://form.jotform.us/form/43168390436155 is associated with a different account.

    Please do check if you are logged in to your account, you can see it by clicking the Account settings icon at the upper right of the page. If you are logged in, you should see your account details.

    I would also like to recommend that you clear your browser's cache and try using different web browsers.

    Hi,
    I have CPMCInformatics set up for my work, who I am creating the form for.
    Would the different account be under my personal account
    anjulidroll@gmail.com.
    Other than that, I cant recall another username I've created.

    Hi,

    I see that this form ID 43168390436155 is now with your account cpmcInformatics. Are you still having problems with seeing your forms? I see forms named "Dragon License Allocation - WBR", "Dragon License Allocation - WBR" and "Communication Request Form" are active and in your account and it seems that you are currently updating them? Are you still having problems in accessing the forms? Let us know if you still need assistance on this.

    Hi,
    I just saved the form as a template and then logged into the CPMCaccoutn
    and pulled down the template. Sorry if the work around was not appropriate
    - you're now welcome to delete the template if you want. Thanks so much for
