    I checked the sample calculator you've shared with us, it's a bit complex but it is doable in the form builder using the form calculation wizard. Have you tried it with your form yet?

    From your sample, it uses radio buttons with different number values on each options. That's something you can configure on the radio button properties on the form builder. You can represent calculation values for each options under radio properties. Check this screenshots as example.
