    Single Choice survey with double rows?

    Hi there,

    I'm trying to make a single choice survey with two rows, is that possible?

    What I want should look something like this:


    but with the blank spaces being the circles to choose.

    It sounds like our "scale rating" tool under the survey tools in the form builder is what you are looking for.

    Hi David,

    Unfortunately as you can see, the result is really messy and hard to understand at a first sight (this is for an important college work, so the presentation matters).

    It'd be ideal to be able to customize the numbers at the top (at least for other numbers). Is that possible?

    It's possible to change the scale of the numbers on the scale rating tool, for example, in my demo form the numbers go from 1 to 10.
