Can i create a uniform order form?

I would like to add in a uniform order form section to my form.  I would like individuals to be able to select a particular item, select a size and then also select a quantity. I would like to be able to do this all in one section/line if this is OK.   Is this possible?

For your specific task, you will need to learn how to add options to your order form -- the options being size and quantity.
