Hi,
You can see here in our Terms how we define Phishing.
It is stated that:
If a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately
I see that your account was autosuspended, this is because our Anti-Phishing system detected one of your forms having a "Password" field, as our terms mentioned, we do not allow forms that acts as login pages or collects sensitive information.
