I changed my email notification address for this form (followed all instructions to the tee) and then I stopped getting any notifications when someone fills it out. The form shows up in the JotForm 'Submissions' location for my account... but the information is no longer being emailed to me.Page URL:

I cloned your form my end. Cant see anything wrong with your configuration.
Have you added the firstname.lastname@example.org address to your email contacts list to be sure the submission emails arent getting dumped into a spam folder?

Are you still unable to receive the email notifications? I made a test submission then checked our mail logs and the results shows that the email was sent out.

Have you checked the spam and bulk folders? The email might have been caught by the Spam Filters. You can provide the logs above to your hosting provider to find out what happened to the message.
