I'm getting a notice that my forms are not found. Why is this?

I made CV survey but it dosent work

I tried  many time but it dosent work, its appear ( form not found this form is disable)

What is the problem?

How can I make it work?

Our apologies for any inconvenience which may have been caused. Our system automatically suspends accounts which contains jotforms it suspects of being used for phishing purposes. Unfortunately, it doesn't do a very good job with foreign language forms. Anyway, I've checked the jotform which triggered the action and have found nothing indicating it's being used for phishing. As such, I have whitelisted the jotform and re-activated your account.
