Hope to get some help
I have created a number of forms and linked then to Infusionsoft using the videos and user manual info.
However the submissions that are going through are not going into CRM database at Infusionsoft.
Any thing that I may have missed.
All I get at Infusion soft is a blank contact form which says a submission has been made but for name it says "info"

It seems like you have removed the integration to the form. When integrating the form to Infusionsoft, you have to make sure that fields are properly mapped to each other.
Please try to reconnect the integration. This time, map only the Name and Email field and then check if that works.
