What is the Sample Contact Form?

I am not quite sure what you mean, but if you are looking to find a sample contact form I would suggest looking at our form templates: http://www.jotform.com/form-templates/category/contact-form

Ben, thanks for your reply. When I typed that question, I had no clue it was going to post immediately.

What I meant was, due to being a new user, I had worked on the Sample Contact form prior to signing up. Once I registered I was taken to the account where I could create a new form. It wasn't until I went back to it, after posting here, that I figured how to navigate back to the design form I had been working on, which is this one http://www.jotform.com/themes/#form/369

It threw me off a bit and now that I was logged in the panel obviously differed. At any rate, I found it; we're good. Thanks again for the reply, it's highly appreciated.
