Hi,
I've checked your form but the background is set to transparent and I did noticed that the text of your form is set to white, please change your font color to black here is the Hex color of black "#00000" in order to see your form text.
Remove this color on your form.
color: #ffffff;
to this color
color: #000000;
Please check this guide on how to inject custom CSS http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes.
Thank you!
