    Hi,

I'm not sure if I understood you correctly, but upon checking your account, I see that you created a form under a guest account, these are the following forms that you have:

    http://www.jotform.us/form/43347395263157

    http://www.jotform.us/form/43346929929168

The second one is actually blank. If you would like to put your form in your website, you can do that by following the guide that we have here: http://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use

In the Embed Form Wizard, you will see different options on how to embed your form to a website, platform or share it on a social media.

I would also like to suggest that you register an account so that you can maximize the use of our form builder.

I hope this helps. Let us know if you need further assistance on this.

    Thank you.

    Hi Robin,

I also suggest that you also signup for an account (for FREE) if you haven't done so yet.

You can use this link http://www.jotform.com/signup to easily create/signup for JotForm account using your email account.

Can you please share to us the URL of your website where we can check the form used?

It will help us better understand the problem if we can check also the embedded form itself.

    Thanks.
