How to use added field text as notification subject

I can't find anywhere to add the text-  Logical Systems Contact Form  so that anytime someone contacts me using the Form that is the text that will show as the Subject in my email box.

I think you are looking to have the Notification Subject labeled per a specified text.

To do this you need to go grab the field tag from the form fields box on the right-hand side of your email notification template editor. Then you would have to use that for the subject line.
