I Created A JotForm Previously & Had Data Collected on a Spreadsheet - I am Now Unable To Find JotForm
The url above is to a spreadsheet where I was collecting data from a JotForm that I previously placed on our church's website www.theroadumc.org. The JotForm is no longer listed on the website and I am unable to find the form!!!
Based on the information in the url above, are you able to retrieve the JotForm associated with this spreadsheet? If you can retrieve the JotForm, would you kindly place it in "My Forms" folder?

Unfortunately, we cannot find out the form details by seeing the integrated google spreadsheet. It seems the form name was "The Road UMC Availability Survey".
Can you please let me know on which web page of your website the form was embedded? I did try to find out if you have any other account but it seems you only have one account. Did you use any other email address in creating JotForm? Do let us know and we will try to trace your form.

You are correct, the name of the form is The Road UMC Availability Survey. The form was embedded on www.theroadumc.org. I created the form in my current JotForm account, not as a guest.
I think that I locate the embed link: It appears to be a Google form: Is JotForm and Google Forms related?:

You are right, the link points to the Google Form. Unfortunately, we cannot convert the Google Form into the JotForm.
You will need to recreate the form using available JotForm tools to see it on your My Forms page.
