I have integrated a form to salesforce.com but not all form submissins are flowing through.
The ones that are are not triggering the lead assignment rules.
Have I missed any settings?

Okay so basically not all of your submissions were sent into salesforce then?
I found your jotform on your account currently contains only 5 submissions.
Can you please tell us which of those submissions didn't go into your salesforce account?

The only two that went through were jack Robinson and Rick O'Shea. The other three have not appeared and I didn't get an error message.
I can't see any common link to the ones that were successful or not.

I can't seem to find any problems with your configuration and my test went through just fine.
I have cloned your form here and integrated it to my SF account and then run 5 consecutive tests. All went through.
Here's the test I've made just now.
I have cleared your form cache here, would you mind re-integrating your form to Salesforce and run a test again? This might somehow help. By the way, just FYI, if you have made changes on your form during that time, re-integration is also required.
