on november 20th my email notifications stopped working. the one that says email@example.com
i have tested the efax service manually and it works

Yes. I see that emails are not being sent and getting failed since November 20th. This is happening with your efaxsend email address on various forms.
As I notice the emails are being sent using your email address following SMTP method. This seems to be an issue with either the originating email address or efaxsend blocking emails from your email address. We request you to check with the SMTP settings on your account and with efaxsend if is blocking emails from your email address.
