Form shows different on my website than it appears in Jotform

I have a box at the  bottom of the form that asks for name and date.  It has shown correctly until I did some editing of other items today, and now it just shows as a full line, no hint on to tell them to put name and date and it is the full length, not just a short spot.

What can I do to get that back?

After selecting a form on My Forms page click "More" and then select "Revision History". So you can revert it to a previous version.
