'Please take short first' error message when trying to submit the form

When I submit my forms I keep getting error message that says "Please take a shot first" and nothing more. The form will not submit.

I have not changed any of the forms but they will not submit, they just say "Please wait".  On another form we don't get an error message we just get "Please wait" but it never goes through.

That's because you have a Take Photo widget to your form. The widget will ask your permission to access your camera then you will have to take a photo of yourself before you can submit the form. For example, see the screenshot below:
