Many customers filling out our form are having difficulty and receiving a script error message after attempting to submit. It only seems to be for one of our forms as of now. The last time it worked for someone to submit was Nov. 26th. Thanks for your help!

I was able to submit to your form. It's taking me a few seconds, but it doesn't show me any error:
If possible, please ask for a screenshot when the error appears so we'd know what script is not working to the form and so we can forward it to our developers.
For now, try to re-save the form into our form builder. After that, clear your forms cache: https://www.jotform.com/help/229-How-to-Clear-your-Form-Cache and re-save the form again. Check if that helps.
Lastly, use our iFrame method if the form is embedded to your website: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Just to make sure that a possible conflict is not causing this.
