How do I add a terms and conditions link into a form?

How do I add a link to Terms and Conditions with a dropbox saying - 'Have you read and do you agree to our terms and conditions'?

There are two related widgets that can be used on the forms:

- Terms & Conditions

- Short Scrollable Terms

Alternatively, the link can be added with a Text field.

The link can be also included into the Dropdown label.
