It appears as though you may have been using the wrong link to clone the form and that it has run into a bug. Occasionally when users clone a form using a link that takes you to our log in page instead, they clone what appears to be a log in page. This in turn causes our filter to suspend your account.
I cleared out the bugged forms and activated your account. Also, here is the proper link to clone the form you are trying to clone:
If the form is in your account, you can also simply clone it straight from there rather than using the URL:
