I need to create a survey for a school that we do business with. The school is a nonprofit educational institution. The questionnaire will go to the parents of the enrolled students, in an effort to gather feedback on our teachers, staff and executive administrators. Once the form returns can the return box be blocked. The results have to be confidential because there will be internal staff tallying the data. Is that possible?

Hi Gwen, by 'can the return box be blocked?' do you mean that any potential user of the form can only submit the form once?
If so, this type of functionality is, unfortunately not available in JotForm at the moment. You could probably technically limit one submission to one PC by adding some clever coding, but there would always be a way round that, e.g a parent could just use a different PC/IP address.
As regards, the anonymonity of your survey, so long as your form isnt capturing any information that could be used to identify the respondent, e.g a name, any results would be anonymous.
