I have taken care of your account issues and upgraded it to premium monthly subscription. Please sign out from your account and then sign in again for the changes to reflect.

I am not sure how many form and submissions you had earlier in your account earlier.
Current status is that you have two forms in your account where "Nezáväzná prihláška" has 5 submission records and "Záväzná prihláška" with 0 submissions.

all my forms are definitely OK, again. Perfect! :)
I am 55 and its firt time in my life, that my registration and upgrade looks like this! Am I too old, or system "plays" with me the "GAME"? Too gloomy :(
