Okay, I just found out that leaving the "Take Photo" widget empty will cause the problem. I've tried re-creating your form and the issue remains the same:
Let me forward this thread to our developers so they can check further. For now, make the take photo widget as required so someone can't skip it or remove it while we are investigating the cause and use the upload field instead.

Thank you for sending on to developers. We've used the form for months with no issues. We allowed two options (uploading file or taking a photo) for the various types of submissions we need from our staff. I just tried clearing my cache in both JotForm settings and also my Chrome browser and the problem persists so it must be the "take photo" problem you mentioned.

Hi thank you for reporting the issue and apologize for the inconvenience that have caused you. We made an update to the widget an the bug should already fixed now.
Please confirm if it works from your end otherwise report it back so that we can further investigate the cause of the problem.
