How do I create a payment form?

I need to create a form that has product selections with prices and quantities. When the user makes a selection the quantity and prices add up and show a total. Any ideas or existing templates like this? It does not need to have a payment gateway like Paypal for example. This is basically a staatic order form to be emailed for fulfillment.
