I have attempted test submissions 2 times and I have not gotten notifications to my email. This started on December 8 when a client submitted info and it never got to me.Page URL:

I see that your email address is in bounce list so you were not able to receive any notifications. I've removed it from the bounce list and have sent you a test submission which was sent successfully.
You can also check if your email is in bounce list and remove it by following the guide below :
