    Easy. I actually did it 15 mns ago.

    Create form in jotform

    Then:        preview

    In the preview window, click on the top right button: “form source”

    Copy the source (text in the withe window)

    Go to muse (in the page you want your form to appear) (design mode)

    Go to Object --- Insert HTML

    And paste what you copied from jost to that window. Et voila.

    Actually. let be more precise:

    Then click: preview      (THE PREVIEW BUTTON ON THE JOSTFORM PAGE.  --NOT--  THE PREVIEW BUTTON ON MUSE!!!!)

    In the jostform preview window, click on the top right button: “form source”  

    Select and copy the source (text in the white window)

    Go to muse (in the page you want your form to appear) (design mode)

    Go to menu :     Object --- Insert HTML

    And paste what you copied from jost to that window. Et voila.
