Phantom Image Appears

I created a form and placed a jpg image in it.  The form appears fine in Jotform.  However, when I embed the form on our website with an iFrame, there is a box that says "Sample Image" that appears at the top of the form.  This should not appear, and I don't know where it's coming from.  Is it possible that there is something embedded in the image that is appearing?  I checked with our website provider and they tell me that it's in the form and they cannot help to fix the problem.  I would greatly appreciate any assistance that you can provide.
