Form not Displaying on Webpage

My forms are still not showing up on the webpage. After a few minutes trying, it gives an error message that they cannot be found. It would almost seem that the pingdom is measuring uptime for the principal jotforms site, perhaps where this forum and other informational pages are held - but it is not measuring the server where forms are held. That server seems to be experiencing a significantly longer downtime that what pingdom is revealing.

Does anyone else have forms showing up on their sites?
