We recently have a denial of service attack or DDOS that we were able to traced to originate from IP addresses from Brazil.
For this reason, we had to ban some IP addresses range from Brazil. Your IP must have been affected by the disabling of the the IP addresses.

I have now elevated your issue to our higher team support so that they can isolate your IP and fix the issue.
Can you please try also adding to your Whitelist the JotForm IP addresses.
Check if this makes any difference.
We will let you know here when we have update on the status.

We're very sorry for the inconvenience. There was a cyber attack from Brazil last week, and we unfortunately had to restrict many of the computers coming from Brazil. This limitation should be lifted shortly, I believe by tomorrow. Thank you for your patience and understanding while we bring everyone's forms back online.

We have received the confirmation from our back end team that the Brazil IP addresses are now enabled. That should have resolved the issues you were having.
