All my forms are not working

    All of our forms are not recording the information registered.
    After you click submit the page goes blank and does not update the database
    Appears only the link and the page is blank.
    What happened?

    I made a test submission using the form you have included and the data was posted in the Submissions section of your account. I was redirected to the correct thank you page url as set in the form response. In addition I also received the notifciation the auto responder set.

    Are you still experiencing this problem from your end?
