After checking the forum for "Form Disabled", it looks like my account may have been suspended because I created a form with a field labeled "password". I have since corrected this.
The goal was the use the form for internal company use to provide external users their credentials to a test account.

Yes, if that is true (that you have created a label "Password") then that would be why your account was disabled.
After removal of the same label however your account would get enabled in about a minute or so, but that would require not only to delete the jotform, but to also Purge it which would permanently remove it from your Trash folder.
