Can I create a form that will add individual product pages to my website?

I do the website for a dachshund rescue. Each dog has their own individual page and I would like a form to fill out for each dog and use it as the dog's individual page.

Let's say this is one the pages and the form was placed there. You would like all submissions made there to indicate that that's the page from which they came. If this is correct, inserting our Get Referrer widget would make this possible, as the notification you receive will show the page's URL.
