Jotform shows disabled message after submission

    After filling out the form and clicking on Submit, there is a response of "Form Disabled."

    How do I correct this?  I sent a previous question with a url that could only be viewed by City of Jacksonville employees.  I have now placed the jotform url below.

    I see that you appear to be having a problem with one of your jotforms being disabled. 

    I checked and tested your jotform " " this on my end but so far everything appears to be normal as I am unable to replicate this issue at the moment.

    Are you using just your jotform url or is it embedded on a website?

    I have also just now cleared your form caches for you to refresh your jotforms for you.
