This is very strange, but on our site the form won't show up unless i look at the site through a proxy server like anonamouse.org. Then it shows up.
Otherwise it just doesn't show anything in that space.
Would our IP be blocked somehow?
We're just using the basic single-line script to embed.
I was able to view the form without going through a proxy server. There could be a few reasons why you are experiencing this issue.
Are you currently trying to access the page on a work network? If so, your network could be blocking the elements of the form.
