Hey!I am trying to make my form collect a zip code from a user. Once the zip is gathered, i want that to forward to a page specific to that zip code.
EXAMPLE:
user enters 45459 for zip code and completes the rest of form. then redirects to custom url: http://www.napower.com/710370/rates-and-plans/select-provider?zip5=45459&customerType=0&x=137&y=13
(NOTE THE ZIP IN THE URL above.) that zip code will be different upon each user that fills the form out. Is there a way of doing this with jotform at all?

Hi,
You can do this by getting the field name of the zip code field and inserting it to your custom URL. You can follow the screenshot below. I've cloned your form for testing purposes.
1. Click the Properties of the Zip Code field of your form and get the name of it.
2. Navigate to the "Thank You" page setting and choose Custom URL.
Based from the link you have above, this is how it will look like.
The {zipCode} is the name of the field where the value will depend on the users input.

Easy enough it sure did! Thanks so much for the answer here! I guess the only thing i need to do is set up commercial so when they click a different box, it does the second string of urls by zip. (im assuming i can use same methods and just add conditions to make this work.)
Thanks again, and Jotform is THE BEST! It's the only place i use for contact forms. :)
