Create Custom Thank You / Landing Pages By Zip Code

I am looking to take my form and when the user submits zip code, the url or landing thank you page would be landed based by their zip.

Ex. If user entered 45421 for their zip code, the form would redirect upon submnission to xyz.com/zip-rates-45421

Is this possible and whats the best way of achieving this? my landing pages already exist for these zip codes, just need to format this form to work with em!

! I know adding conditions would work but I would have to manually add WAYYY to many zip codes to urls. It would take FOREVER!
