Hi, A few nights ago I logged onto jotform and a survey pop-up appeared. It asked if I would take the 15min survey--if I did it would record my voice and watch my screen actitivity. I accepted and then realized what the hell I'd probably just done-- allowed a hacker/bad guy to use my comp. So I logged off of everything, rebooted my comp, and did a virus/ad sweep.
I am writing to ask if this was a legitimate survey from Jotform or if it was malicious.

I am pretty sure it wasn't from JotForm, JotForm doesn't provide any Advertisements or Malicious Content. So yes! its actually a Virus or Spyware which may harm your computer or collect some of your personal informations, such as Internet surfing habits and sites that you've been visited.
And, glad to know you were able to handle the case. We should always be careful in most of the pop-up pages as they always bring viruses.

Apparently, there was a survey being conducted by JotForm a few days ago as discussed here - http://www.jotform.com/answers/46828-Today-s-Jotform-Survey-and-25-Gift-Card. So I believe this is a legitimate survey commissioned by JotForm.
But as what idarktech advised, please exercise caution whenever you see pop up advertisements in general to avoid your data and privacy from having compromised and exposed to hackers.
