Hi,
I checked our logs for your form and one of the addresses was having problems receiving their notifications. This may have been the result of an issue we had with our mail servers last week. Since the emails were not being completed, this resulted in one of your addresses being placed in our bounce list:
I have removed the address from the list and the notifications should be able to be sent to them once again.
