We have just received an empty question form you with just an image. Is the issue related to the error message you see when you use your form's SSL URL?
I did check your form and form some reason it has css code to import css which is not valid. The file name is not complete. Please check the screenshot below:
Did you edit your designer injected custom css code manually? If not edited manually, please explain the changes you did in form designer so that we can replicate the issue here.
For now, I have edited your form and deleted the invalid custom css code. That seems to have solved the issue and it will not display any error message in console.
