Checkboxes shown as checked on page load on GoDaddy forms

I created three forms, all of which the check boxes are pre-populating with a checkmark on my website.  How can I set these up so that the default is unchecked boxes?

    Hi,

Can you please give us a bit more details?

If you call the jotform without any parameters sent to it, it will open empty - unselected checkboxes. If you specifically send them the parameters to get pre-filled it will check those options, so default would be to not select them.

It would be useful if you can tell us the steps that one would take on your website to see the jotform (since you mention a checkbox on your website) and also to include what you would like to happen once they come to jotform.

    Best Regards,
    Ben

The only parameter I set for the jotform is making the checkbox field a "required" fields.  Once a user is on my website, they simply click my "forms" tab which takes them to a page that has the jotform embedded.  It is still showing each checkbox as "selected" rather than "unselected"

    Hi,

I took a look at these 3 pages under 'Forms' tab:

    http://discoveringstrengthpt.com/Client_Registration.html

    http://discoveringstrengthpt.com/Par_Q.html

    http://discoveringstrengthpt.com/Lifestyle_Information.html

    even the contact page: http://discoveringstrengthpt.com/Contact.html

Clicking on the 'Forms' tab itself seems to just open the home page.

None of the pages had jotform on them and we can only assist you with forms (jotforms) created with JotForm Builder. I believe that the ones you have on your pages are of GoDaddy so I would need to ask you to contact their support instead.

If the issue is with a jotform, please give us a link to the jotform on your website and we will take a look at what is going on and be happy to assist you in correcting the issue.

    Best Regards,
    Ben

Ok, it is fixed now.  Thanks.

One more question.  Next to each box you see "option 1 2 and 3.  I have deleted those options several times and they keep reappearing.  I simply want a single checkbox to show.  How can I fix this?
