I am unable to log in with my email

I have reset my password four times and it continues to say my username and password do not match. I need to get logged in ASAP.
Username is lennis@mb2dental.com.

Thanks for reaching out to Jotform Support. I have checked your account, and it is auto-suspended due to your Form ID 230175041106037, which was asking for login information. Login forms are not allowed. Please review our Terms of Use. You need to remove or delete the form that violates the terms before we can request the reactivation of your account.

I tried to reset my password multiple times but the link never comes to my email. Now it’s saying my IP address is blocked. Can someone please tell me what this means and what I can do to solve this ? Thank you!

That makes sense. But how can I remove the form if I can’t even login to get to the form?

Thanks for getting back to Jotform Support. I have deleted the login credential fields from your form and reactive your account. Please refrain from using the fields which violate Jotform Terms of Use. You can now log in to your account.
