I have created a student progress report form. How do I set it up so that the form is filled out by teachers, and then emailed to the parents?

Can you tell us if the parents will be added to the jotform - their names and emails, or would the teachers need to type all of them before they send it out?
Based on what you have said, I presume that you would like to implement the same thing described here as well: Set Email Field as the Notification Sender Email Address with Mandrill
Basically what you will see is how to send an email to the email address that is located in the email field - added by someone.
I would however suggest that you add the emails to the jotform as they are easily typed out wrongly when people do not add their own emails.

Thank you for your answer. I will look at the link and see how to configure this situation.
Regarding your first question, I would like the teachers to type the parents email and info in to the forms.

I looked at the thread that you sent and it looked...tedious, complicated, and time consuming.
The solution that I came up with was to put the form on a hidden page on the website, allowing them to login, complete the forms for every student. When they hit submit, the form comes to the JotForm inbox, where I then forward the form to the parents email.
Now the question is:
How do I put my logo on the form, rather than JotForms?

It seems you are forwarding the submissions from your form's submission page. Please be noted that we have limits where in free account you can only forward 2 submissions per hour.
As you are anyway inputting the parent's email address to forward submissions, there is a better way to achieve your requirement. Here are the steps you should follow:
1. Add an parent's email field in your form.
2. Add a "Form Autoresponder" in your form to send submission email to the parent's email. The following guide should help you: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder

If I understand your question correctly, you want to change the JotForm logo you see in the submission emails. Is that correct?
You will have to edit your form's email alert template and then change the logo URL. Please check the screenshot below:
