How do I put my logo on the form, rather than JotForms when forwarding the form via email?

I am not sure if I have understood your question correctly. Do you want to add an image in your form or replace the JotForm logo from the submission emails you receive?

To replace the JotForm logo from the submission emails, you will have to actually change it in the email template of your form. Please check the screenshot below:
