Have you tried adding the Responsive mobile widget to the form?
If you are in the normal Form view on the left hand side Select Widget and in the search box type Responsive you would see the Widget appear just select it and it is installed in your form.
If you are in Design view go to Form Layout and select Make this Form responsive.
Not a good idea to use both options .
Hope this helps
