when i go to submit entry, ti says its unavailable. it is make enabled.

I did check your form and I am unable to replicate the issue you are having. I was able to submit your form successfully without any problem. Please check the screenshot below:

You may also like to take a look at your form's "Form Limit" settings as well where you have set "Nike Email" to be a Unique question. That means if you use the same email address second time in the "Nike Email" field, your form will not accept the submission.
Please check the screenshot below:
If you are getting this error message that means you are using the same email address twice to submit the form.
