I would like to create an approval flow

I would like to create an approval flow for change management requests. The idea is the request is submitted for approval, once approved id like the form to be sent back to the sender with more areas to complete (Implementation, Testing, Closure and attach files). Once that is completed Id like it to continue through the approval process for a second review and approval by my change advisory board. How can I do this with the approvals workflow? I can see where I can send another form, but can I make visible hidden fields on the same form?

We really appreciate your patience and understanding while we’re looking into this. Your requirement is possible but you'll have to set up conditional logic to the approval workflow as well as to the form in order to achieve it.

But if you want to make it simple and easy, you can just create another two forms and add the fields that a user needs to complete once it is approved.
