I can't upload files on the form.

I checked your form and I was able to replicate the issue. I tried uploading jpg, png, or pdf but I'm still getting the same error. I tried using Mozilla, Internet Explorer and Chrome, still having the same issue. I cleared your form cache, but it is still a no go.
However, when I cloned your form and it is working properly.
I will forward this issue to our developers so that they can take a look at it. We will notify you via this thread once an update is available.

We sincerely apologize for the inconvenience this caused you.
The server error you got when you were attempting to upload files was caused by a full server hard disk on our side, which has since been resolved.
I notice you removed the upload fields from your form, you can now restore them, and they will work as expected.
